Before filing a claim, make
sure that the insured on whose behalf you’re filing the claim is
protected under the insurance coverage.

N.B.: Original invoices and
receipts for the requested reimbursement must be submitted to the Company
within 90 days following the accident.
Certain accidents may be covered or certain
expenses may be reimbursed under a group insurance plan or a government
agency such as:
If such is the case, the claim must first be submitted to the agency concerned,
and a copy of the claim settlement must be enclosed with the claim.

form (PDF – 59 KB).
