The policy covers the medical conditions described below:
Cancer
A malignancy, which is characterized by the uncontrolled growth and spread of malignant cells and the invasion of tissue.
Exclusions
The following forms of cancer are excluded: non-invasive cancer in situ and any skin cancer except invasive malignant melanoma into the dermis lower than a depth of 1.0 mm.
Deafness
Permanent and profound loss of hearing in both ears, without any possibility of correction. The auditory threshold must be more than 90 decibels.
Kidney Failure Permanent irreversible failure of both kidneys which necessitates treatment by dialysis.
Major Organ Transplant The irreversible failure of the heart, liver, bone marrow, lungs, pancreas or kidneys requiring a transplant of that organ, resulting in the insured being operated on or accepted into a recognized transplant program.
Paralysis The complete and permanent loss of use of 2 or more limbs without interruption for a minimum period of 90 days.
Cystic Fibrosis The diagnosis of cystic fibrosis.
Blindness Permanent and irreversible loss of sight in both eyes without any possibility of correction. The corrected visual acuity must be at least 20/200 or less in both eyes or the field of vision must be less than 20 degrees in both eyes.
Cerebral Palsy A cerebral motor condition causing significant mental and physical incapacity which prevents participation in physical and school activities.
Muscular Dystrophy A diagnosis of muscular dystrophy characterized by muscle weakness.
