Before filing a claim, make sure that the insured on whose behalf you’re filing the claim is protected under the insurance coverage.

Death or Loss of Limb Claim

	Contact the insured's representative. or
	Call one of the following numbers, between 8:00 a.m. and 5:00 p.m. (Eastern time), from Monday to Friday: - Quebec City region: (418) 684-5332 - Other regions (toll free): 1-888-715-5232

N.B.: Original invoices and receipts for the requested reimbursement must be submitted to the Company within 90 days following the accident.

Certain accidents may be covered or certain expenses may be reimbursed under a group insurance plan or a government agency such as:

	the Commission de la santé and de la sécurité du travail (CSST);
	the Société de l'assurance automobile du Québec (SAAQ);
	the Régie de l'assurance-maladie du Québec (RAMQ); or
	the Victims of Crime Initiative (VCI).

If such is the case, the claim must first be submitted to the agency concerned, and a copy of the claim settlement must be enclosed with the claim.

Disability Benefit Claim

	Print, complete and sign the claim form (PDF – 114 KB). The second page must be completed by the attending physician.
	Return the form, along with the necessary supporting documents, to the following address: Industrial Alliance Insurance and Financial Services Inc. Life and Health Claims (IRG 2506) 1080 Grande Allée West PO Box 1907, Station Terminus Quebec City, Quebec G1K 7M3
