product that provides plan members and their family with the
financial peace of mind they need in order to deal with their condition
and focus on recovery.

Member’s Critical Illness Insurance
|
This coverage guarantees the payment of the
lump sum benefit provided for in the contract to an insured who survives
30 days following the diagnosis of a covered illness. It is offered with
a choice between units or multiples of annual salary.
The group plan covers the following critical
illnesses:

Member’s Optional Critical Illness
Insurance |
This additional Critical Illness Insurance amount
is offered with a choice between units or multiples of annual salary.

Spouse’s Optional Critical Illness
Insurance |
This Critical Illness Insurance guarantees that
plan members will receive payment of the lump sum benefit provided for
in the contract if their spouse survives 30 days following the diagnosis
of a covered illness. The coverage is offered with a choice of units.
The group plan for spouses covers the same critical
illnesses as the Member’s Critical Illness Insurance
plan.

Children’s Optional Critical Illness
Insurance |
This Critical Illness Insurance is especially
tailored for children. It guarantees that plan members will receive payment
of the lump sum benefit provided for in the contract if their child survives
30 days following the diagnosis of a covered illness. The coverage is
offered with a choice of units.
This amount could allow plan members to take
time off in order to care for a seriously ill child or to cover expenses
related to a child’s illness.
