Filing a Claim

Before filing a claim, make sure that the insured on whose behalf you’re filing the claim is protected under the insurance coverage.

Claim Request

	Contact the person in charge of the insured’s group. or
	Call one of the following numbers, between 8:00 a.m. and 5:00 p.m. (Eastern time), from Monday to Friday: - Quebec City region: (418) 684-5332 - Other regions: 1-888-715-5232.

N.B.: Original invoices and receipts for the requested reimbursement must be submitted to the Company within 90 days following the accident.

Certain accidents may be compensated or certain expenses reimbursed by a group insurance plan or a government agency such as:

	the Commission de la santé et de la sécurité du travail (CSST)
	the Société de l'assurance automobile du Québec (SAAQ)
	the Régie de l'assurance-maladie du Québec (RAMQ)
	the Victims of Crime Initiative (VCI)

If such is the case, the claim must first be submitted to the agency concerned, and a copy of the claim settlement must be enclosed with the claim.

Disability Benefit Claim

	Print, complete and sign the attached claim form (PDF – 552 KB). The second page must be completed by the attending physician.
	Have the form signed by the person in charge of the group.
	Return the form, along with the necessary supporting documents, to the following address: Industrial Alliance Insurance and Financial Services Inc. Life and Health Claims (IRG 2506) PO Box 1907, Station Terminus Quebec City, Quebec G1K 7M3
