Making a Donation Request
1. Make sure you meet all the eligibility
requirements.
2. Write us a letter outlining your request and
describing the organization you support.
3. Submit your donation request:

Complete and print the PDF
donation request form (PDF – 133 KB)
and attach it to your request.

Send your letter and the completed form to the following address:

If you have any documents that describe your organization
in more detail, you may attach them to your request. Please
do not send videos or voluminous documents.

After an initial analysis, the person in charge of processing
your request will contact you if more information is needed.
|
|
Because of the large number of donation requests we receive,
only properly completed requests will be considered.
